Add custom fields to the widget contact form

Allow admins to add structured fields to the built-in widget contact form, including text inputs and dropdowns. For example, a logistics support team needs a load-number field and a severity dropdown so each request includes the details needed to identify and triage the issue. Changing the form description or asking visitors to include these details in the message is not equivalent to collecting them as separate fields. Keep the submitted field values with the support request.
